Failures by a Tampa assisted living facility to follow its policies helped lead to an 81-year-old man’s death a state investigation found, according to the Tampa Bay Times.

Maynell Scott, who had Alzheimer’s, walked out of Belvedere Commons on July 16 and was found three days later in a lake near Fletcher and North Armenia avenues, the Times reported.

It was not the first time he had wandered away and according to a report from the Agency for Health Care Administration, he should have been flagged as an escape risk and fitted with a GPS wristband, the Times reported.

The facility’s failure to follow that policy and others led to Scott’s drowning, the agency found.

Originally founded in December 2006 as an independent grassroots publication dedicated to coverage of health issues in Florida, Health News Florida was acquired by WUSF Public Media in September 2012.
